Application Form

By agreeing to the terms and conditions of this project, you accept that you have given full use of your submitted photos to Jacinta Lo Nigro (Cinnah) for her year 12 Studio Arts workbook, folio and final piece.

This includes:

1. Giving full artistic licence of your images to Jacinta Lo Nigro (Cinnah) (i.e.: Experimentation with submitted images, alteration, not creating exact likenesses etc)
2. Use of images to be suitable to my uses and choices.
3. The display of any work related to these images,
4. And possible submission and acceptance into The VCE Season of Excellence: Top Arts.

You also agree to:

1. The inability to change any product of this work, even if you don't personally like it
2. The inability to withdraw yourself from this project once work with your images has been finalised (doing this could collapse the whole thing if you do this!)
3. Full co-operation and correspondence with anything relating to and with retrieval of information and possible additional photo requests.

You agree that if you have filled in this form on behalf of a friend, that the reasons are not malicious, and you have their full approval of you doing so.

If you have filled out this form falsely in attempt to annoy me or make a joke, you agree to my full use and manipulation of whatever photos you have submitted in ANY way, and manipulation of any photo of YOU if I discover your identity.

I personally reserve the right not to use any image submitted to me, and cannot assure the images created from your faces will be in entire likeness.
